The Care Transition Coordinator (Ref: CT/ACG/26/4) role includes work on weekdays, weekends, public holidays and on-call on a rotating basis. Key duties include managing schedules, business proposals, client assessments, and insurance reimbursement filings. Candidates must be able to work under stressful conditions, stand for long periods at heights, lift persons and materials, and perform other activities related to the role. Successful candidates must be able to work both as a live-out or a live-in based on the clients' requirements. Short-notice assignments are required.
Requirements
Registered nurse with a Bermuda Nursing Council license or international jurisdiction equivalent, Bachelor's degree or higher. Supervisory experience in healthcare environment preferred. Mandatory homecare experience as an RN or caregiver. Experience with Electronic Medical Records (CareSmartz360 preferred). At least two recent satisfactory employment references, valid police background check, and Bermuda driving license required within 4 weeks of employment. Compliance with non-smoking policy and random substance testing.
