Key responsibilities include providing social services and discharge planning, performing bio-psychosocial assessments, and communicating with patients, family members, and care providers to establish a care plan.
Requirements
Master of Social Work Degree (MSW) from an institution accredited by the Council of Social Work Education (CSWE). Two (2) years of post graduate experience preferably in a healthcare setting. Consideration will be given to person with experience in multi-disciplinary team approach. Pre-employment substance abuse screening is mandatory.
