Key responsibilities include: Serving as nutrition consultant to the health care team(s) and providing direct and indirect nutrition care to patients Identifying and assessing patients that are at nutritional risk, patients on therapeutic diets and patients with complex medical conditions for individual needs Monitoring patient's progress for efficacy of nutritional intervention, provides reassessment of nutritional status, and adjusts nutrition care plan according to established protocols Providing counselling and education to patients/significant others regarding drug-nutrient interactions, nutrition intervention or therapeutic diets as appropriate. Responding to nutrition Consults from physicians and the health care team and initiates the nutrition care process Promoting the team approach through participation in patient care rounds and discharge planning meetings Qualifications &/ Registration required: Bachelor or Masters of Science from an accredited University with a major in Nutrition/Dietetics with a minimum of one-year post internship experience in a clinical hospital setting, within the past three years Local registration to practice with the Council for Allied Health Professions (CAHP) and meet all re-registration requirements Must demonstrate ability to provide independent clinical nutrition care services to a wide range of patients with simple to very complex medical nutritional therapy Commitment to clinical standards of care, personal and professional growth, and improvement of Dietetic services within the hospital Excellent interpersonal and communication skills; liaison with other members of the multidisciplinary team to ensure continuity of care Must be proficient in the Microsoft Office Suite Allied Health Services participates in a six-day rota that includes evenings, weekends and public holidays. Pre-employment substance abuse screening is mandatory for all successful candidates.
