Candidates must offer the following: • A related Bachelor's Degree and/or the equivalent of three years of experience in case management and behavior management. • Maturity and strong interpersonal, case management, and family engagement skills are essential. • Experience delivering youth group programmes, well developed written and oral communication skills, and creative thinking capabilities. • A passion for being a change agent in a person's life and a commitment to working in a culturally sensitive and supportive manner at all times. • Willingness to work flexible hours and to participate in continued professional development. • A valid Bermuda Driver's License, a safe driving record, and willingness to obtain a Bermuda community service driver's license. Major duties and responsibilities include: • Carry out screening and intake new families. • Conduct home observations and provide home coaching on an ongoing basis to help families practice newly learned strategies. • Respond to case management needs for increased stability within the family by assisting in problem solving with various life skills, child advocacy needs, home and practical support. Connect families to resources, as necessary. • Attend school team meetings, request school reports, and conduct school observations on a consistent basis. • Maintain individual client charts in a timely manner following the agency procedure for documentation of client services. • Effectively communicate with service team members to determine clients' needs, status and goal action planning. • Utilize dynamic risk assessment guidelines for individuals and families in programmes and activities to ensure safety. • Work in collaboration with other professional agencies in the best interest of clients. • Co-facilitate therapeutic groups and related activities for children and families. • Take part in team planning and delivery of therapeutic groups. • Assist in carrying out performance quality improvement tasks to enhance service delivery.
