Mid-Atlantic Wellness Institute (MWI) is the sole psychiatric hospital in Bermuda. We provide hospital and community-based care. Under the direction and leadership of the Clinical Manager, the Community Psychiatric Nurse (CPN) will engage and support clients/patients and their families in the community, including clients’ homes. Key responsibilities include: Conducts mental health assessments/triage of the presenting symptoms of individuals referred to Outpatient Services; Manages assigned case load, conducts assessments and triage of referred patients/ clients, provides support to Clinic Triage Nurse (CTN) and supervises the work of support staff as applicable; Administers medications to patients/clients; Orders medications for patients/clients; Encourages support staff to deliver person-centered care that enables patients/clients to engage in meaningful activities and relationships; Engages members of the public in the promotion of mental health/wellbeing and relapse prevention; Acts as Case Manager for assigned clients or groups of patients to develop and implement treatment plans and programs; Liaises with caregivers, relatives, and other care providers for best outcomes of patients/clients.
