Bermuda Hospitals Board is hiring a Under the direction of the Clinical Manager, the Clinic/Psychiatric Triage Nurse will undertake and perform the assessment and triage process of all referrals to the acute out-patient clinic. Key responsibilities include: Providing an assessment / triage of the presenting symptoms of individuals referred for service to the acute out-patient clinic; Ensuring that the management of the out-patient clinic is efficiently coordinated and interfaces seamlessly with the continuum of care; Ensuring that services to be given are achieved in the most efficient and effective manner commensurate with sensitivity to clients’ needs; Reporting changes in the clients condition or behaviour to medical staff and/or other disciplines as appropriate within the parameters agreed with the multi-disciplinary team; Maintaining the required standards of documentation and adheres to the protocols for managing medical records; Ensuring responsibility for the preparation of scheduled psychiatrist/psychologist out-patient clinics, wherein all pertinent clinical information is made available and all subsequent requests for follow-up actions are completed; Ensuring compliance with required safety practices within the workplace as well as the community; Facilitating partnerships with community agencies related to care delivery.Registered Mental Health Nurse with a minimum of 4 years post graduate experience in psychiatric nursing, an additional year of experience in a community mental health setting is preferred; Certification in CPR, Code Management and NCI; Ability to quickly assess and make critical decisions regarding medical or psychiatric patients; Excellent verbal and non-verbal communication skills when interacting with clients, family members and staff; Good interpersonal skills; Registered with the BNC, possessing membership in good standing with the appropriate professional organization.
